Goal: Healthy Love seeks to provide a safe, culturally tailored intervention for heterosexual black women to reduce their disproportionately high risk of transmitting and contracting HIV and other STDs. Healthy Love aims to encourage sexual abstinence, HIV testing, and receipt of test results; increase women's condom usage during vaginal sex with male partners; and reduce the number of women's sex partners and unprotected anal and vaginal sex with male partners. Healthy Love also seeks to improve HIV/STD knowledge, self-efficacy for using condoms, intentions to use condoms, and attitudes towards condoms.

Impact: Healthy Love increased participants' likelihood of using condoms, being tested for HIV, and receiving their test results. The intervention also reduced participants' self-described actions with male partners that can increase black women's risks for HIV infection.

Goal: HeartStrings aims to enhance the quality of life for individuals with special needs. Through regularly-recurring, interactive sessions, the MSO’s Rhapsodie Quartet addresses the physical, emotional, cognitive, and social needs of hundreds of individuals each year in a comfortable and familiar setting.

HeartStrings mission statement is as follows:

1) To enhance the quality of life of underserved populations through live, interactive, and exceptional quality musical experiences that are informed by the American Music Therapy Association’s Standards of Practice.

2) To bring meaningful arts experiences directly to participants in a comfortable and
familiar setting.

3) To provide a valuable resource for facilities that serve aging populations, adults with dementia, and individuals with disabilities, or long-term illnesses.

Impact: HeartStrings has reached over 3,200 individuals with disabilities, long-term illness, and assisted-living needs, as well as aging adults with dementia since its start in 2006.

Goal: The Illinois WISEWOMAN program (IWP) aims to lower heart disease and other chronic disease risk factors through screening and lifestyle classes for women in high-risk populations in service counties throughout Illinois.

Impact: The Illinois WISEWOMAN Program addresses the disproportionate risk of cardiovascular disease among disadvantaged, low-income women. Participation in the program has been shown to improve dietary, physical activity and cardiovascular outcomes.

Goal: To reduce drug abuse and increase positive mental and physical health outcomes among college students ages 18-25 years old.

Impact: Tailored health and wellness interventions may reduce risk factors facing college students, while perhaps improving their health-related quality of life.

Goal: Addressing environmental issues on a watershed basis requires the cooperation and participation of many different people. This initiative aims to facilitate partnerships between state agencies, NGOs and civil society in order to yield better environmental results.

Goal: The goal of It's Your Game: Keep It Real is to reduce teen pregnancy, prevent STI transmission, and delay teen sexual activity in middle school students.

Impact: Participants in the It’s Your Game: Keep It Real intervention program were less likely to initiate sex by the ninth grade when compared to the control group.

Goal: Research shows that children benefit from kinship care in many ways. Kinship care can reduce the trauma that children may have previously endured and the trauma that accompanies parental separation by providing them with a sense of stability and belonging in an otherwise unsettling time. Children who have been placed with relatives may have experienced chronic neglect and physical, sexual, or emotional abuse. While these experiences place children at risk for behavioral and health problems, a positive relationship with a caregiver and a stable and supportive living environment can mitigate their impact.1 Grandparents, other relative caregivers, and “fictive kin” — close friends holding a family-like bond with a child — are in a unique position to fill this supportive role and promote resiliency.

The goal of Kinship Connections is to support kin families' social, emotional, and economic needs to increase placement stability within the child’s community. Specific program objectives are to improve family economic security, family relationship functioning, child well-being, and to increase kin caregiver social support.

Goal: The program aims to provide information and resources to kinship caregivers (grandparents, relatives, foster parents, and others) in New York State.

Goal: MOVE goals are to visually demonstrate health inequities and positive changes in Washington State, to highlight local Communities Putting Prevention to Work efforts to improve health, and to foster local partnerships.

Impact: The MOVE initiative is empowering community members to identify and raise awareness of the health inequities impacting them.

Goal: To increase daily fruits and vegetables servings by half in women served by WIC participants with the long term goal of reducing risk of cancer.

Impact: The Maryland WIC 5-A-Day Program shows that while multi-faceted community based interventions can effectively promote and sustain dietary change among low-income populations in order to reduce the risk of cancer, many obstacles remain in implementing such programs.
