The goal of the MEND program is to reduce obesity levels in children by offering free healthy living programs that aim to encourage small lifestyle changes that improve health.
The MEND program was successful in reducing waist circumferences and BMI scores while increasing cardiovascular fitness, physical activity, and self esteem in children placed within the intervention group. The Goal of this Promising Practice is to determine whether a tailored community health worker (CHW) intervention would improve post-hospital outcomes among low-SES patients.
This intervention would improve access to primary care and quality of discharge while controlling recurrent readmissions in a high-risk population. Health systems may leverage the CHW workforce to improve post-hospital outcomes by addressing behavioral and socioeconomic drivers of disease.
