Community Health Improvement Plan 2023-2025

 

TidalHealth, Somerset County Health Department (SCHD), and Wicomico County Health Department (WiCHD) worked collaboratively to develop this Community Health Improvement Plan (CHIP) and Implementation Strategy in response to the 2022 Community Health Needs Assessment. Healthy Delmarva’s partners develop an Implementation Strategy/Community Health Improvement Plan every three years based on the priority areas identified in the Community Health Needs Assessment. This plan explains details about the strategies and activities the partners will use to address public health issues.
